InnoTraction has configured a solution to address the specific needs of the emerging Home Telehealth (HTH) industry. Home Telehealth is used to monitor and manage individuals with chronic illness. Telehealth enables the remote acquisition of vital signs and symptoms and the delivery of educational content in support of self-management. Telehealth monitoring and interventions are managed by clinicians following best practice protocols.

Many starting points exist for HTH programs. InnoTraction has developed a HTH maturity model which we discuss with clients during strategic planning workshops.
Using the InnoTraction Maturity Model (IMM) we guide clients through the pros and cons of varying degrees and combinations of automation and patient engagement to arrive at the ideal level at which to begin their HTH program.
